At Integrated Dx, it is our desire to assist in improving patient care by increasing access to affordable Diagnostic Laboratory testing services. That is why we have created our patient friendly billing policy known as the Integrated DX Financial Assistance Program.

 

Because each case is unique, our Financial Assistance Program team will work with patients to help them understand their bill and, when applicable, how to receive assistance with their patient obligation. For Medicare or other government programs, as required by law, we bill the patient or their secondary insurance for any co-pay, deductible, or co-insurance amount

  • Financial Hardship
    • Eligibility for the Financial Hardship Plan is determined by your income level and the U.S. Department of Human Services poverty guidelines. You may find the guidelines at US Health and Human Services
    • To apply for our Financial Hardship Plan, download an application and mail it to the address listed on your invoice, or fax to (855) 704-1599.
  • Interest Free Payment Plan
    • Can be offered for in-network, and out-of-network patient responsibility
    • For the patient to be eligible for an Interest Free Payment Plan, the patient (or patient guardian must) contact the Billing & Collections department by phone or in writing (mail or e-mail) within 30 days of receipt of their bill requesting a payment plan
    • Credit card information is obtained to process payments on a monthly basis for duration of payment plan.

No, Integrated Dx’s Billing Department will suggest a minimum payment, but ultimately it will be set by what the patient is most comfortable with.

Integrated Dx will send a reminder letter of the agreed payment plan after 1 month of missed payments.

Payment is accepted in the form of check, money order, and credit card payment. Checks and Money Orders should be made payable to Integrated Dx and mailed to PO Box 4080, Winchester, KY 40392-4080

Yes, the income is accounted for on a household basis. If a patient is married, then both the patient and spouse’s income must be provided and verified. If a patient is married but separated, then only the patient’s income is needed.

Integrated Dx will have the patient fill out the Indigent Patient form and indicate that their income is zero and attest to their statement with their signature.

Integrated Dx’s main priority is to remain compliant with all laws and regulations regarding proper billing. However, if a patient is deemed to not be eligible for a discount, Integrated Dx will still be able to offer assistance to the patient via Option number 1.
