Understanding Your Bill

Integrated Dx is a team of medical diagnostic experts, committed to efficiency and cost-effectiveness in clinical laboratory services. A physician providing you care recently used our services for laboratory testing and results. You have received a separate bill for that service.

Please contact the ordering physician or medical practice to review your results.

Payment is accepted in the form of check, money order, and credit card payment. Checks and Money Orders should be made payable to Integrated Dx and mailed to PO Box 4080, Winchester, KY 40392-4080

We rely on the referring physician, healthcare facility, or you to provide us insurance information to submit your bill for a claim. If that information has not been provided, you will see the full cost reflected. Once insurance information is provided, we will submit your claim and provide you an updated statement for any remaining sum that your insurance has not covered. 

We will work with you to resolve any billing issues and ask that you contact our team to discuss Integrated Dx’s financial assistance program to help you navigate the patient’s portion of the bill.

A deductible is the amount you pay each year for most eligible medical services or medications before your health plan begins to share in the cost of covered services. For example, if you have a $2,000 yearly deductible, you’ll need to pay the first $2,000 of your total eligible medical costs before your plan helps to pay.

A copay (or copayment) is a flat fee that you pay on the spot each time you go to your doctor or fill a prescription. For example, if you hurt your back and go see your doctor, or you need a refill of your child’s asthma medicine, the amount you pay for that visit or medicine is your copay. Your copay amount is printed right on your health plan ID card. Copays cover your portion of the cost of a doctor’s visit or medication.

Coinsurance is a portion of the medical cost you pay after your deductible has been met. Coinsurance is a way of saying that you and your insurance carrier each pay a share of eligible costs that add up to 100 percent.
